LEMTRADA REMS Program Pharmacy Enrollment

  • Pharmacies must be enrolled in the LEMTRADA REMS Program to be able to dispense
    LEMTRADA for patients with multiple sclerosis.

To enroll in the program, an authorized representative of the pharmacy must complete the following steps:

  1. Designate an authorized representative
  2. Register the authorized representative with the LEMTRADA REMS Program Training Center
  3. Authorized representative must review the LEMTRADA REMS Program Overview
  4. After reviewing the material, complete and sign the LEMTRADA REMS Pharmacy Enrollment Form.
    This enrollment must be renewed every 2 years
  5. Implement the necessary staff and training processes to comply with the LEMTRADA REMS Program requirements
